A Change Management Plan defines the roles and activities needed to implement and manage change. It is useful and important to prepare a Change Management Plan, even if it is simple, to help think through the steps and level of effort needed to make the change.
The Change Management Plan includes four phases: Initiate, Plan, Execute, and Maintain. Let’s take a closer look at the phases and the activities involved with each one.
