Overview
PCMH Programs
Since the PCMH concept was introduced, PCMH recognition, designation, and accreditation programs have been developed. Each program varies slightly in its interpretation of the PCMH model. The Medical Group Management Association has published a comparison of the programs. The following list contains National PCMH Recognition and Accreditation Programs:
- National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition program
- Accreditation Association for Ambulatory Health Care (AAAHC) Medical Home On-site Certification
- The Joint Commission Designation for Your Primary Care Home
- Utilization Review Accreditation Commission (URAC) Patient-Centered Medical Home Accreditation
